Background:Viagra (sildenafil) is a popular drug used to treat erectile dysfunction (ED) and pulmonary arterial hypertension (PAH). It is also prescribed to improve sexual function in patients with cardiac diseases such as heart failure and hypertension. The drug has also been found to reduce the risk of stroke in patients with heart failure, particularly in patients with pre-existing heart valve disease. This study evaluated whether sildenafil is safe and effective in reducing mortality in patients with heart failure with PAH, with the aim of determining whether the drug is safe and effective in this group of patients.
Methods:Patients with chronic heart failure (CHF) and PAH were enrolled between June 2013 and April 2014. Inclusion and exclusion criteria were age, duration of CHF, duration of PAH and duration of ED, and a history of any cardiac disease. Patients were evaluated for eligibility and assessed for comorbidities and other clinical variables, including age, sex, concomitant medications, and concomitant therapies. The study was approved by the institutional review board of the University of Zagazig University of Medical Sciences (approval number: IRB number: 14-1082-001-002). Inclusion criteria were patients with an eligibility score of ≤ 6 and at least 2 chronic cardiovascular comorbidities (such as hypertension, arrhythmia, or other). Patients were evaluated for the presence of cardiovascular comorbidities and other clinical variables. Patients were randomly assigned to receive either sildenafil 100 mg daily or placebo. The primary outcome was mortality at the first visit. The secondary outcome was the presence of pulmonary arterial hypertension (PAH), defined as the presence of at least two of the following cardiovascular comorbidities: hypertension, angina, myocardial infarction, or stroke. The primary efficacy outcome was the occurrence of death in the study group at the first visit.
Results:The final number of patients was 31, of which 34 (83.4%) were in the sildenafil group and 31 (83.4%) in the placebo group. The mean age was 63.3 (SD ± 5.5) years in the sildenafil group and 67.7 (SD ± 5.9) years in the placebo group. The most common comorbidities were hypertension (43.7%), arrhythmia (28.5%), and myocardial infarction (23.7%). The most frequent treatment-related comorbidities were diabetes (23.7%), hypertension (21.1%), and hypertension (9.4%).
Conclusion:The results of this study suggest that sildenafil may be more effective in patients with heart failure with PAH, with a higher reduction in the occurrence of pulmonary arterial hypertension, compared to placebo. The study provides further evidence that sildenafil may be more effective in patients with heart failure with PAH.
